Testimonial and Review Approval Form

I hereby grant Xpressdocs Partners, Ltd. and its affiliates (“Xpressdocs”) permission to use the testimonial and photographs, video, or other digital media (“photo”) I’ve provided in any and all of its publications, including web-based publications, without any additional future payments or other considerations.

I hereby irrevocably authorize Xpressdocs to edit, alter, copy, exhibit, publish, or distribute the testimonial for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein the likeness of my testimonial appears.

Additionally, I waive any right to royalties or other compensation arising or related to the use of the quoted testimonial.

I hereby hold harmless, release, and forever discharge Xpressdocs from all claims, demands, and causes of action, which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

By filling out and submitting this form, I acknowledge that I have read and understand the terms and conditions above.
